Child Support Agreement Illegitimate Child

AGREEMENT FOR CHILD SUPPORT

[NAME] THE MOTHER, referred to as MOTHER, and [NAME] THE FATHER, referred to FATHER, agree:

FATHER acknowledges paternity of a male child born on [DATE] in ______________, [CITY] ___________ [STATE] named _____________________.

The parties are not married to another nor have they been married to one another.

The child's name after the execution of this agreement shall be ______________________. Upon request of FATHER, MOTHER shall cooperate with any administrative or other proceeding to effect such name change. Such proceeding shall be at the sole expense of the FATHER.

FATHER agrees to pay child support in the sum of $------- (dollars) per month, on or before the 15TH date of each month.

The sum of child support shall be adjusted on the anniversary of each payment to account for any increase in the consumer price index, cities. In no event shall the payment decrease from year to year.

The child shall have the right to inherit from the estate of the FATHER.

Such support shall continue until the child reaches the age of 18, dies or marries.
