The franchiser, _________, and the franchisee, ___________, as set forth in their franchise agreement on ________ and as witnessed by ________, have agreed to the following additional terms and conditions:

Any order placed by the franchisee for __________ will be done so on specific order form developed and provided by the franchiser. Date of orders will be ______ as agreed by both parties.
