1. Introduction. [Name], being duly sworn, deposes and says:

2. Description of Deponent. I, [name], have never changed my name or used any other name [or, prior to my marriage to [name], I was known as [name]]. I am a citizen of the United States and at least eighteen (18) years old. After today, I will live at [address].

3. Ownership and Possession. I am the only owner of property located at [address] (Property). I now sell the Property to [name] (Buyers). I am in sole possession of the Property. There are no tenants or other occupants of the Property. I have owned the Property since [date]. Since that time, no one has questioned my ownership or right to possession. I have never owned any property that is next to the Property. Except for my agreement with Buyers, I have not signed any contracts to sell the Property. I have not given anyone else rights concerning the purchase of the Property.

4. Liens. No liens, encumbrances, or judgments have been created against my ownership or use of the Property. No bankruptcy proceedings have been started by or against me. No security interests have been created against any personal property or fixtures included in this sale. No easements or licenses have been granted against the Property. All liens listed on the attached judgment search [omitted] are not against me but are against others with similar names.

5. Improvements to the Property. [Description of improvements] have been made to the Property since [date]. All municipal charges have been paid in full. I am unaware of any mechanic's lien that has either been filed or is intended to be filed against the Property. I have had no notification that money is owed for any construction or repair work on the property.
