1. Consideration. This Release is being executed by Releasor for the consideration of the sum of ---------- dollars ($----------), receipt of which is acknowledged.

2. Subject Matter of Release. The Estate of [name] hereby releases Releasee, Releasee's heirs, assigns, and personal representatives from all claims or causes of action arising from [description of accident] that resulted in the death of [name].
